In an attempt to reduce the IN-PERSON TIME COMMITMENT REQUIRED FOR COMPLETING FORMS for BOTH our clients and our staff, as well as for your convenience, we have created fillable PDF documents which provide you the opportunity to have the required form(s) completed before your appointment.

NOTE: There is no requirement to have these forms completed before you come in for your appointment. If we have not received them before your arrival, we will fill them out in your presence. If you need to send them only partially filled out, let us know in the email and we will finish them together.
